COVID-19 Evaluation and Testing
If you are experiencing COVID-19 symptoms or have been exposed to someone with COVID-19, we are available Monday – Friday to assist you. We are currently offering PCR testing by appointment only with results in 2-3 days. To schedule an appointment please call our office. Common symptoms include:

- Shortness of breath
- Fatigue
- Muscle or body aches
- Headache
- Congestion or runny nose

You can download and fill out our New Patient Information Form and relevant Pre-Visit Questionnaires. Our accepted payment and insurance information is also listed. We require payment for services in full at your appointment. It is the responsibility of the patient to pay any copays or deductibles.
